The Exceptional Family Member Program (EFMP) is here to help your family successfully navigate life’s many transitions—whether you’re facing deployment, a Permanent Change of Station (PCS), a new diagnosis, retirement, or permanent dependency planning.
EFMP helps families with special medical or educational needs identify and prioritize concerns through the Family Needs Assessment. Also called DD Form 3054, this standardized planning tool is a living document to be updated as your family’s needs change. It is available across the services to provide your family-focused and consistent EFMP family support, regardless of where the military may take you.
At the heart of EFMP’s support is a collaborative approach to goal setting. EFMP Family Support Staff work closely with you to identify needs and create a personalize plan using the Family Needs Assessment. This process helps clarify priorities, set achievable goals, and connect you with the right resources every step of the way.
No concern is too big or too small. Share what you are comfortable with, and your EFMP team will help build a strategy tailored to your family’s unique journey.
